Protocol for Oxford Out of Town Patients

Doctor Likover has numerous patients coming from outside of Houston for the Oxford procedure.
This instruction sheet will explain how our office cares for these patients.

Office Hours:

Monday through Friday, 8:30 a.m. until 5:00 p.m. Closed Saturdays, Sundays and Holidays.

Please bring the following items with you to your appointment.

1. Your insurance card or other health coverage information. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt.
2. If you are having your knee examined, we suggest that you wear walking shorts.
3. Any existing X-rays or MRI of the area to be examined. Please check out your actual MRI films or X-rays and bring them with you.
4. Medical records from your referring physician or current treating orthopedist that relate to your orthopedic condition.
